Welcome Home Program

Returning home from a stay at the hospital or the Larsen Health Center can be stressful. Wouldn’t it be nice to have the assistance of a caregiver to help make the transition home as smooth and comfortable as possible? Through the Welcome Home program, a resident may receive two hours of home care services provided by Shell Point Home Care’s experienced caregivers.

The nursing team also offers a host of personalized services to meet the varied and diverse needs of every resident. This includes coordinating and communicating healthcare services between residents, family and providers; coordinating medical appointments and information; offering and accompanying residents to medical appointments; and providing patient advocacy, emergency crisis management, intervention and support.
